Obtaining a Birth or Death Document

Immediate family members with proper identification are authorized to receive certified nativity and death certificates. Authorized agents must present proper identification and acceptable documentation in society to obtain certified certificates. To obtain a birth or death certificate, you must have the following information:

Total name of the person named on the certificate
Date of birth (required for birth certificates only)
Father'southward full name (required for nativity certificates merely)
Female parent's full maiden name (required for birth certificates simply)
